Insurance Information
Columbus Ophthalmology is a provider for most major insurance companies. The following are a list of many of the insurance companies that we participate with. If your insurance company is not listed, please call your insurance provider to make sure we are covered on your plan. It is important for you to bring your medical insurance card and a referral if needed for HMO policies.
We understand that the cost of health care concerns our patients. That is one reason why we accept Medicare Assignment, which means that we agree to charge no more than what Medicare says is an allowable charge. Please remember, however, that Medicare will pay only 80 percent of that charge. Patients are responsible for the remaining 20 percent. If you have a good supplemental or "Medigap" policy, it will usually cover the difference.
As part of our effort to hold down costs, we ask patients to pay for routine exams and treatments at the time of service. For our patients who are enrolled in various Health Maintenance Organization plans, all you need pay is the co-payment required by your particular plan. If your insurance requires a referral, please make sure you have called your primary physician before the appointment to obtain it. If the referral is not here, you will have an option of signing a waiver, rescheduling your appointment or paying for the visit in full.
